Dear Parent/Guardian
To prepare for the upcoming school year, we are asking you to kindly review this letter carefully and fill out the appropriate forms.
Have your medical care provider complete and return to the Health Office on or before the first day of school. Please make sure to keep a copy of your paperwork for your records. Failure to submit completed paperwork will result in your child(ren) being excluded from school-sponsored field trips and activities.

Students requiring medication, other than the above-mentioned, must submit the form(s) below (click on the links), which must be filled out by the parent/guardian:
Note: The Grade Level Physical Form can be used for a physical that you can have your medical care provider fill out when you go to your pediatrician. If you have any questions, do not hesitate to contact us.
